Sunday Golf Tournament
1:00 p.m. start, 4-person scramble format. Sunday, September 21, 2008 at TPC Scottsdale Golf Course. The $110.00 fee includes, greens, cart, box lunch, beverages and tournament prizes. Transportation to and from the course is available.

The bus leaves from The Fairmont at 12:00 noon on Sunday, September 21, 2008. The $75.00 fee includes fishing license, guides, prizes, box lunch, and beverages. Please bring sun protection.

SUBSTITUTION/CANCELLATION POLICY
Substitutions are accepted at any time. To obtain a refund of your registration fee (minus a $300 processing fee), you must notify Prepared Foods by emailing bogdajewiczj@bnpmedia.com or call 1-630-694-4353 and receive a confirmation of that email before 4 pm EST, August 8th, 2008. No refunds will be granted after that deadline.
